How to set up an appointment with me?

You would need to sign up at Mdlive.com and create an account as an MDLIVE patient. Then search for my profile and send me an appointment request with your day and time preference through the website. You can send a request even if it shows that I don’t have availability. I’ll review your request and answer.

Does MDLIVE take insurance?

Yes, for the most up to date information you would need to call (800) 400-MDLIVE for insurance and payment information.

Important information:

1. I am only licensed to practice in the state of Nevada. That means if you travel out of state you would need to wait to make an appointment until you are physically back at state of Nevada.

2.Once you become a MDLIVE patient we can only have communication through mdlive.com which is a HIPAA compliant website and not this website.

3. MDLIVE charges a $50 fee if appointments are not canceled within 24 hours.
